Participant Forms & Consent

Purpose
This page covers parent and guardian consent, participation risks, and emergency medical information for KairoLink Foundation programs. It explains what consent means, what risks are involved in athletic participation, and how medical information is collected and used.
Who May Provide Consent
Consent for a minor athlete must be provided by a parent, legal guardian, or authorized adult with responsibility for the athlete. KairoLink may request additional verification when needed to protect the athlete or confirm authority.
Application Consent
By submitting an application for a minor athlete, the parent or guardian confirms that the information provided is accurate, that KairoLink may use it for eligibility and safety review, and that KairoLink may contact the parent or guardian about the application.
Program Participation
Acceptance into a KairoLink program may require additional written consent, waivers, emergency contact details, medical information, attendance commitments, academic expectations, media choices, transportation terms, and conduct expectations.
Assumption of Risk
Soccer and athletic activities involve inherent risks, including falls, collisions, overuse injuries, sprains, strains, fractures, concussions, heat illness, dehydration, illness exposure, equipment-related injuries, field conditions, transportation risks, and other unexpected events. Parents, guardians, and athletes acknowledge these risks as part of participation.
Parent and Athlete Responsibility
Parents, guardians, and athletes are responsible for sharing relevant medical, allergy, medication, emergency contact, and participation limitation information before activities. Athletes should follow coach instructions, use proper equipment, report injuries, and avoid participating when unsafe.
Emergency Medical Information
KairoLink may request emergency contacts, allergies, medications, known medical conditions, physician information, insurance details, and physical limitations relevant to athlete safety. This information is used to plan safe participation and respond to emergencies. It is treated as sensitive and accessed only by authorized people.
Emergency Response
If a participant appears injured, ill, or in danger during a KairoLink activity, authorized adults may seek emergency medical assistance, contact emergency services, notify parents or guardians, and share relevant medical information with responders.
Medical Clearance
KairoLink may require medical clearance or restrict participation if there is a reasonable safety concern, incomplete information, recent injury, illness, or activity limitation. Parents and guardians should keep emergency and medical information current and notify KairoLink promptly about changes.
Communication
KairoLink may communicate with parents or guardians by email, phone, text, or other reasonable methods about applications, program updates, safety matters, scheduling, academic requirements, and foundation operations.
Withdrawing or Updating Consent
A parent or guardian may contact KairoLink to update information, withdraw an application, or request that optional consent such as media consent be changed. Some records may still be retained for legal, safety, or administrative reasons.
No Guarantee of Acceptance
Providing consent or submitting an application does not guarantee program acceptance, financial assistance, travel, college placement, playing time, sponsorship, or any specific outcome.
Release of Liability
To the fullest extent permitted by applicable law, participants and their parents or guardians release KairoLink Foundation, its board, staff, volunteers, coaches, mentors, contractors, and partners from liability for injuries, losses, or damages arising from program participation, travel, events, or related activities, except where caused by gross negligence or willful misconduct.
Separate Waivers
Certain activities may require a signed waiver, travel consent, code of conduct agreement, or other document before participation. KairoLink will provide these when applicable. Failure to complete required documents may result in exclusion from the activity.
Insurance
KairoLink does not provide personal health insurance for program participants. Families are responsible for maintaining appropriate health coverage for their athletes. KairoLink may carry general liability or event insurance where required.
Indemnification
Participants agree to indemnify and hold harmless KairoLink Foundation from third-party claims arising from their own conduct, violations of program rules or applicable law, misrepresentation, or unauthorized actions during KairoLink activities.
